With regards to the issuance of various forms of certification such as a certificate of graduation, etc., please see the attached Issuance of Certificates Request Form (please note the forms differ between academy, and university graduates). After filling out the appropriate application form, please enclose the below documents and send the required cash amount via registered mail.

1. Issuance of Certificates Application Form
2. For university graduates and academy graduates, a handling fee of 300 yen (cash) applies per certificate.
3. Proof of identity (drivers license, a copy of your health insurance card)
4. Please ensure the non-standard sized envelope is self-addressed and the required stamp is attached for the return mail (envelope with dimensions 216mm x 277mm - Kakugata No.3)

A few days are required for the issuance of the certificates, so please apply well in advance. Moreover, the issuance of English certificates can take around 2 weeks.
